When it comes to our loved ones, we always want peace of mind they are safe in their homes or while out and about living the life they want.

Delta Wellbeing can help put your mind at ease through our CONNECT service which is FREE for 6 months.

The pilot project, which is the first of its kind in Wales, has been funded by the Welsh Government’s Transformation Fund, enabling the West Wales Care Partnership to work together to help shape the future of health and social care services across west Wales.

As part of CONNECT, we offer a full wrap-around support service for you or your loved ones throughout CarmarthenshirePembrokeshire and Ceredigion. This includes flexible Technology Enabled Care (TEC) support packages, pro-active wellbeing calls, digital support, help to re-engage with the community and access to an appropriate Community Response Service 24/7.

How do I sign up for FREE?

Step 1: Contact Us

Firstly, you would get in touch with one of our friendly Advisors on 0300 333 2222. This could be a self-referral, via a family member or through a social care or health professional 

Step 2: Wellbeing Assessment

We would undertake a Wellbeing Assessment to identify what level of support may be required and establish priorities areas, such as food shopping, loneliness, social interaction or managing a health condition.

Step 3: Pro-active Wellbeing Calls

The assessment determines the frequency of follow up wellbeing calls to support your welfare – this would depend on your specific needs.

Step 4: Wellbeing Activities & Digital Support

We also support wider wellbeing activities, such as re-engaging with the local community (virtually currently, but physically in the long term). This could be identifying and providing support to attend community groups of interest such as gardening or dancing groups or by connecting like-minded individuals through the CONNECT 2U app.

We would also provide you with a GDS Digital Tablet and protective case. A router with a 4G sim card can be included if you do not have an internet connection at home. The Tablet includes a variety of apps such as games, latest news, exercise and health information.

We can an also provide apps to video call or Whatsapp friends and family. We can help set this up and keep track of the battery life letting you know when it’s low.

Step 5: Community Responder Service

We also provide a 24/7 Community Responder Service when in crisis supporting non-injurious falls and welfare needs giving you and your family peace of mind knowing help is always available.
